Social Security number for you and any other members of your household
Verification of income for the last month. If working, we will need a pay stub
If you are married we will need his stub as well
If you receive child support and/or alimony, we will need those totals
Bring any current health insurance: name, policy, and ID numbers
If in house applying- Please allow up to 1 hour for your appointment

If you need a copy of your birth certificate and you were born in LA, call or stop in at the Health Unit at (337)480-2670 from 8:00A.M.-3:00P.M.
